The forms/waivers linked below must be turned in on the first day your child attends camp. Please fill out before arriving for a faster check-in process. We will have copies available in-person at drop-off.

 One set of forms is needed for each child and for each camp your child is enrolled in. For example, if you have 1 child who is enrolled in Elementary Camp week 1 and in Playground Camp week 2, each camp would need its own set of completed forms.
